Key Responsibilities

  • Conduct adult/geriatric assessments including medical history and treatment
  • Administer and order point of care and lab-based testing
  • Collaborate with healthcare professionals and family members
  • Provide services at members' residences
  • Document care according to Matrix guidelines

Required Qualifications

  • Master’s Degree or commensurate experience
  • Current NP/PA licensure in state of practice
  • NCCPA, AANP, AACN or ANCC board certification
  • Current BLS, ACLS or CPR certification
  • 1 year of NP or PA experience
  • Valid state driver's license
  • Strong computer skills

Preferred Qualifications

  • Ability to obtain prescriptive authority
  • Excellent verbal and written communication skills
