Job Description

Key Responsibilities
Plan, organize, and deliver physical therapy programs
Conduct examinations, evaluations, and interventions for clients
Document client information for evaluation and establish therapeutic plans
Educate clients and family members about treatment plans and self-exercises
Demonstrate knowledge of age-related treatments and evaluations
Participate in departmental activities and promote the organization
Required Qualifications
Licensed as a Physical Therapist in the state of practice
Appropriate education level in accordance with state licensure
Basic Life Support (BLS) certification or obtained within the first 60 days of employment
Preferred Qualifications
Valid driver's license
Experience in home health care
