As a Therapist - Physical Therapist, you will have the opportunity to provide high-quality care to patients in the comfort of their own homes, making a meaningful difference in their recovery and well-being.

Overview

As a Physical Therapist with our Home Health team, you will be responsible for conducting patient assessments, developing and implementing individualized treatment plans, and providing evidence-based care to patients with a variety of physical disabilities and limitations. You will work collaboratively with our multidisciplinary team to ensure seamless care coordination and exceptional patient outcomes.

Required Skills

• Current licensure as a Physical Therapist in the state where the position is located
• Valid BLS certification
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• Ability to adapt to changing patient needs and priorities
• Basic computer skills and ability to use electronic medical records

Nice to Have Skills

• Experience working in home health or rehabilitation settings
• Knowledge of evidence-based practice guidelines and treatment protocols
• Ability to communicate effectively with patients with diverse cultural and linguistic backgrounds
• Certification in specialized areas such as pediatrics, geriatrics, or orthopedics

Preferred Education and Experience

• Master's degree in Physical Therapy or related field
