Key Responsibilities

  • Engage members to complete comprehensive needs assessments
  • Develop and implement person-centered care plans
  • Partner with internal care teams and community resources
  • Provide education and coaching for member self-management
  • Support proactive discharge planning and care transitions
  • Advocate for members and families to ensure needs are met

Required Qualifications

  • 2 years of case management experience serving members with Serious Mental Illness (SMI)
  • 1 year of case management experience serving elderly and/or persons with physical or developmental disabilities
  • 1 year of experience with MS Office (Word, Excel, Outlook)
  • Access to reliable transportation

Preferred Qualifications

  • Bachelor’s degree in Psychology, Special Education, or Counseling
  • CCM certification
  • Experience working in team-based care
  • Experience in Managed Care
  • Social Work experience
  • Bilingual
