Key Responsibilities
Engage members to complete comprehensive needs assessments
Develop and implement person-centered care plans
Partner with internal care teams and community resources
Provide education and coaching for member self-management
Support proactive discharge planning and care transitions
Advocate for members and families to ensure needs are met
Required Qualifications
2 years of case management experience serving members with Serious Mental Illness (SMI)
1 year of case management experience serving elderly and/or persons with physical or developmental disabilities
1 year of experience with MS Office (Word, Excel, Outlook)
Access to reliable transportation
Preferred Qualifications
Bachelor’s degree in Psychology, Special Education, or Counseling
