Job Description
Join The Jewish Board as a Case Associate, where you'll play a crucial role in helping clients achieve independence through personalized care and resource management. Collaborate with social workers and make a meaningful impact in the community by providing support and guidance to those in need.
Key Responsibilities
Supply appropriate interventions to ensure client care
Develop action plans to utilize existing and new resources for client care
Conduct routine community contacts and home visits
Evaluate clients' living conditions and independent living skills
Provide individual/group counseling
Perform case management and community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
