Key Responsibilities
Oversee and direct case management staff to ensure coordinated patient care
Manage staffing, policies, and procedures for discharge planning and care coordination
Develop and implement operational plans including staffing resources and quality improvement activities
Ensure compliance with regulatory standards and case management functions
Required Qualifications
Bachelor's Degree in Nursing
Washington Registered Nurse License upon hire
3 years of discharge planning and case management experience in an acute care setting
3 years of recent experience with diverse patient populations including various age groups with physical or developmental disabilities, emotional, behavioral, and mental disorders
Experience working with patients who struggle with substance abuse and knowledge of medication-assisted treatment programs (Methadone/Suboxone)
Preferred Qualifications
Master's Degree in Nursing or Community Health
Coursework/Training in Crucial Accountability Training
Coursework/Training in Emotional Intelligence Training
