Key Responsibilities
Collaborates with Care Transition team for high-risk patients
Coordinates patient care activities with healthcare team and community partners
Intervenes in suspected abuse/neglect cases
Identifies and mobilizes patient and family strengths for resource optimization
Assesses and coordinates family and community resources to support discharge plans
Leads patient-centered conferences
Maintains patient records and participates in discharge planning activities
Conducts psychosocial assessment interviews and revises care plans as needed
Provides patient advocacy and assists with financial program applications
Screens for barriers to care and provides counseling related to palliative care
Required Qualifications
Master's degree in social work/welfare from an accredited school
Two years of social work or case management experience in an acute setting or protective services
Preferred Qualifications
Active certification in Case Management (ACM or CCMC)
Current and valid license as a Clinical Social Worker issued by the State of California Board of Behavior Science Examiners
