Join Caretenders Hospice as an RN Case Manager and make a meaningful impact in end-of-life care. You will develop trusting relationships with patients and families while leading the delivery of compassionate care.
Key Responsibilities
Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient
Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral
Identify and reassess the patient's/family's physical, psychosocial, and environmental needs at least every 15 days
Initiate and coordinate the plan of care while maintaining accurate clinical notes
Document problems, goals, interventions, and patient/family responses to hospice care
Collaborate with the patient/family, attending physician, and other members of the IDG in providing care
Required Qualifications
Current RN licensure in the state of practice
One year of clinical experience
Current CPR Certification
Current driver's license
Valid vehicle insurance
Access to a dependable vehicle or public transportation
