Key Responsibilities
Provide home-based physical therapy to patients with lost mobility, strength, and functionality due to illness or injury
Work as part of a home health team on the patient's plan of care
Observe movements, conduct assessments, document progress, and modify care plan as needed
Assist patients with exercises and hands-on therapy to manage pain and increase mobility
Educate patients on their plan of care, established goals, exercises, and self-care
Coach family on the plan of care, therapy goals, and safely assisting with exercises
Required Qualifications
Graduate of an accredited Physical Therapist Program
Active PT license in FL
Minimum of 1 year of physical therapy experience in home healthcare
CPR certification
Knowledge of state and federal home health regulations
Good clinical judgment
Strong interpersonal skills
