Job Summary:

The RN Care Manager will coordinate concurrent utilization management activities hospital-wide and act as the primary link between the patient, healthcare team, organization, and payors during the patient’s hospital stay.
Responsibilities:
- Conduct patient needs assessments, identify resources for transitions, and secure necessary support.
- Collaborate with, educate, and lead interdisciplinary teams in daily care rounds, care conferences, and care planning.
- Communicate effectively in written, verbal, and electronic formats (email and hospital software applications).
- Coordinate with community organizations for durable medical equipment, insurance authorizations, and transportation.
- Monitor and manage inpatient hospital length of stay.
- Apply knowledge of hospital reimbursement methods and quality measures relevant to care management.
Requirements:
Experience:
- 3-5 years of clinical experience in medical/surgical nursing, home care, hospital case management, and/or utilization review.
- Must have utilization review and discharge planning experience.
Licenses/Certifications:
- Active Registered Nurse license (Associate or Bachelor degree-prepared).
- Active BLS required.
- CCM or ACM preferred.
- Certification by a nationally recognized case management or utilization review organization preferred.
Education:
- Associate or Bachelor degree in Nursing
