Join HarmonyCares as a Nurse Practitioner (PRN) to provide high-quality, coordinated health care in patients' homes. You'll conduct comprehensive health assessments, collaborate with healthcare teams, and maintain patient confidentiality while ensuring excellent care.
Key Responsibilities
Conduct comprehensive in-home health risk assessments
Take patient history and determine diagnoses
Provide written documentation of patient visits per NCQA standards
Take patient vital signs as necessary
Place case management referrals and communicate with PCP as necessary
Communicate with patients, caregivers, agency nurses, and other providers
Perform clinical duties while observing OSHA Universal Precautions
Maintain patient confidentiality
Attend required meetings and participate in committees
Participate in professional development activities
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
