Join CenterWell Home Health as an Occupational Therapist and provide personalized rehabilitation services that empower patients to regain strength, mobility, and independence in their own homes. Collaborate with healthcare teams to optimize patient care and enhance quality of life.
Administer and interpret diagnostic and prognostic tests
Confer with the patient's physician and healthcare team
Provide therapeutic treatment and instruction
Review/expand retraining programs
Consult with vocational team members
Report patient status and progress
Participate in care coordination and discharge planning
Design community reintegration activities
Recommend/design special adaptive equipment
Train staff and family members on retraining programs
Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance
Document patients' care observations and interventions
Required Qualifications
Degree from an accredited Occupational Therapy Program
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
Valid driver’s license, auto insurance and reliable transportation
Preferred Qualifications
A minimum of six months of occupational therapy experience
Home Health experience
