What You’ll Do
- Provide care to patients managing chronic conditions
- Educate patients on treatments, medications, and self-care
- Track and adjust care plans for better health outcomes
- Administer medications, document progress, and respond to needs
- Use clinical judgment and autonomy within correctional guidelines
