Key Responsibilities
Provide assistance and strategies for individuals to live well
Develop meaningful relationships with participants
Help participants with personal hygiene and behavioral challenges
Support participants with developmental goals like budgeting, exercise, and nutrition
Take initiative to help participants engage in community activities
Required Qualifications
Six months of experience in human services, direct care, or care coordination preferred
Valid Driver's license and access to a registered vehicle with proof of insurance
