Join CenterWell Home Health as a Physical Therapist Assistant, where you'll provide personalized rehabilitation care to help patients regain strength and independence in their homes. You'll work under the supervision of a Physical Therapist to administer treatment plans, ensuring high-quality care and compliance with healthcare standards.
Key Responsibilities
Administer the physical therapy treatment plan as developed by the Physical Therapist
Administer therapeutic exercises and modalities to patients
Accurately document client records and submit clinical paperwork on time
Attend educational meetings and contribute to department in-services
Notify physical therapist of any equipment failures or changes
Schedule clients and perform varied office tasks
Apply concepts of infection control and universal precautions in patient care activities
Promote compliance with fiscal intermediaries and/or other third-party payors
Required Qualifications
Associate degree in Physical Therapy from an American Physical Therapy Association
Current state license to practice Physical Therapy
Valid driver’s license, auto insurance and reliable transportation
Proof of current CPR certification
Minimum two years experience as a Physical Therapy Assistant
Comprehensive knowledge of the physical, emotional, social, and biological changes that occur in the geriatric population
Self-directed and enthusiastic
Good verbal and written communication and organization skills
Ability to maintain confidentiality
Must read, write, and speak fluent English
Good and regular attendance

Ability to work a flexible schedule and travel locally
