Job Description
Join Sunshine State Health Plan as a Case Manager, where you'll play a crucial role in transforming healthcare for our 28 million members. This position involves developing personalized care plans for members with mental and behavioral health needs, ensuring they receive high-quality, cost-effective healthcare.
Key Responsibilities
Evaluate member needs via phone or in-home visits
Develop ongoing care plans for members with high acuity
Coordinate care between members, families, and providers
Monitor and adjust care plans based on member status
Provide education to members and families regarding treatment options
Document care management activities for compliance
Required Qualifications
Master's degree in Behavioral Health or Social Work or Degree from an Accredited School of Nursing
2 – 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Experience in pediatric care
Knowledge of behavioral health and social determinant needs
