Schedule: Day Shift | Monday–Friday | 8-hour shifts (8x5)
Pay: Up to $70/hour

Overview:
A reputable healthcare facility in Los Angeles, CA is hiring a Registered Nurse Case Manager for a 13-week contract. This role is ideal for an experienced RN with a strong background in care coordination, discharge planning, and interdisciplinary collaboration . You will play a key role in ensuring patients receive high-quality, efficient, and compassionate care throughout their hospital stay and beyond.

Key Duties:

  • Perform comprehensive patient assessments and develop individualized care plans
  • Coordinate care with physicians, specialists, and support staff
  • Plan and facilitate safe discharges and transitions of care
  • Advocate for patients and their families while navigating complex healthcare systems
  • Monitor outcomes and adjust care plans as needed
  • Maintain accurate and timely documentation in accordance with regulatory standards

Required Qualifications:

  • Current and active RN license in California
  • BLS certification (American Heart Association preferred)
  • At least 2 years of recent RN case management experience , preferably in acute care settings
  • Strong knowledge of discharge planning, utilization review, and insurance processes
  • Excellent communication, critical thinking, and time management skills

Contract Highlights:

  • 13-week contract
  • Day shifts, Monday to Friday (no weekends or nights)
  • Up to $70 per hour, based on experience and qualifications
