Job Description
Join The Jewish Board as a Case Associate, where you'll make a significant impact by serving clients in their communities. This role involves developing personalized action plans, conducting home visits, and facilitating access to essential resources for client care.
Key Responsibilities
Function as a case planner and ensure client care
Develop action plans utilizing existing and new resources for client care
Conduct routine community contacts and home visits
Evaluate living conditions and individual needs of clients
Provide case management and individual/group counseling
Perform community outreach and establish linkages with support organizations
Maintain necessary documentation including progress notes and risk assessments
Required Qualifications
BA Degree
Experience in a community or social service group or five years as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
