What You’ll Do:

• Assess, plan, and coordinate care for patients with complex needs during acute admissions.
• Collaborate with physicians, utilization review nurses, and the interdisciplinary care team to streamline care progression.
• Lead discharge planning, ensuring safe and timely transitions of care.
• Serve as an expert mentor for colleagues, offering guidance on complex care management cases.
• Actively resolve barriers to care and advocate for patient/family needs.
• Provide education and support to patients and their families through every step of the care journey.

What We’re Looking For:

• RN license (Georgia or Compact State).
• Bachelor’s degree in Nursing required.
• BLS certification (Instructor accepted).
• ACM or CCM certification preferred.
• 3+ years acute care hospital experience as a staff RN.
• 2+ years case management experience in hospital or payer-based model.
• Strong problem-solving, communication, and collaboration skills.
