Job Description
Join The Jewish Board as a Senior Case Manager, where you'll empower adults with serious mental illness to achieve independence and improve their quality of life. Collaborate with residents and coordinate support services while advocating for their needs in a compassionate, culturally competent environment.
Key Responsibilities
Carries a complex caseload and practices person-centered service planning
Supports residents in achieving independent living skills
Ensures service plans are completed according to regulatory guidelines
Advocates on behalf of residents with service providers
Facilitates resident support groups and meetings
Completes documentation of resident interactions and progress
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
In-depth understanding of mental illness, substance use disorders, or trauma
Experience working with individuals living with serious mental illness
Knowledge of local and state health and human service systems
Preferred Qualifications
