Job Description
Join Cambridge Health & Rehabilitation Center as an MDS Director (RN) where your expertise will directly impact the quality of care for our aging residents. Lead the MDS program, ensuring compliance and accuracy while fostering a culture of continuous improvement and support.
Key Responsibilities
Manage the Minimum Data Set (MDS) assessments for personalized resident care plans
Ensure compliance and accuracy in MDS assessments
Monitor Case Mix Index (CMI) scores
Train and mentor MDS Coordinators
Coordinate comprehensive resident assessments and care planning
Collaborate with interdisciplinary teams for accurate data collection
Provide ongoing education to facility staff
Analyze assessment data for compliance and optimization opportunities
Drive continuous improvement in nursing care
Required Qualifications
Valid state RN license
Direct care in a long-term care setting and/or MDS Coordinator experience
Proficient in state and federal regulations governing the MDS and billing process
Knowledge of Electronic Medical Record (EMR), PDP, MDS 3.0, Medicaid and Medicare requirements
Deadline driven and detail-oriented
Strong organizational skills and analytical capabilities
Excellent written and verbal communication skills
Ability to work in a multidisciplinary team
