Job Description
Join The Jewish Board as a Case Manager, where you'll empower adults with serious mental illness to achieve independence in the community. You'll lead complex cases, guide residents in recovery, and advocate for their needs within a culturally competent framework.
Key Responsibilities
Carries a caseload and practices person-centered service planning with residents
Ensures service plans are completed according to regulatory guidelines
Evaluates and assesses independent living skills
Advocates on behalf of residents with other service providers
Creates management plans to improve safety and prevent crises
Completes regular documentation of all resident interactions
Participates in staff meetings and trainings
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS)
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
