Job Description
The Case Manager at The Jewish Board provides essential support to adults with serious mental illness, helping them achieve independence through personalized recovery goals and skills development. This role involves direct resident support, administrative coordination, and advocacy, emphasizing a compassionate and culturally competent approach.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning
Ensures service plans are completed according to regulatory guidelines
Supports residents in meeting individual goals including medication management independence
Advocates on behalf of residents with other service providers
Completes regular and timely documentation of resident interactions
Participates in planning and execution of special events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
At least 1-year experience in Human Services preferred
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
Familiarity with Microsoft Office suite of products
Valid driver's license and willingness to drive agency van
