Job Description
Join The Jewish Board as a Case Associate and make a significant impact by helping clients build skills for independent living. You'll work closely with individuals and families to create tailored action plans, provide case management, and connect them with community resources.
Key Responsibilities
Function as a case planner to ensure client care
Develop action plans to utilize existing resources for client care
Conduct routine community contacts and home visits
Evaluate clients for individualized treatment programs
Provide case management and individual/group counseling
Perform community outreach and establish linkages with other organizations
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
