Key Responsibilities
Establish trusting relationships with patients and families
Coach patients in chronic health condition management
Advocate for patients and act as a liaison with community service agencies
Document activities and service plans
Assist patients in accessing health-related services
Facilitate communication between providers
Required Qualifications
Must be 18 years or older
Must live within 30-45 minutes from the service site in Portland, OR
Must have a high school diploma or GED
Must be a U.S. Citizen or Permanent Resident/Green Card holder
Must be able to pass a national criminal background check
Preferred Qualifications
Familiarity with Microsoft Office Suite (Word, Excel, PowerPoint)
