Job Description
Join UnitedHealthcare as a Field Foster Care Coordinator, where you'll use trauma-informed approaches to support children and youth in the foster care system. This role allows for telecommuting within specific counties in KS, providing flexibility while making a significant impact on the health and well-being of families.
Key Responsibilities
Assess, plan and implement care strategies for children and youth
Identify, coordinate, and monitor referrals for services
Manage care plans as a single point of contact
Communicate health-related information to stakeholders
Advocate for children, youth, and families
Provide trauma-informed education
Coordinate with child welfare and juvenile justice systems
Coordinate additional services as needed
Required Qualifications
Master’s Degree in Social Work, Nursing, Psychology or related human services area
Unrestricted, independent licensure as a Registered Nurse
Bachelor’s Degree in Social Work, Rehabilitation, Nursing, Psychology, Special Education, Gerontology, or related human services area
2+ years of experience working within the community health setting in a healthcare role
1+ years of experience working with multi-system children
1+ years of experience working with MS Word, Excel and Outlook
Ability to travel in assigned regions
Access to reliable transportation & valid US driver's license
Preferred Qualifications
Licensed Social Worker or clinical degree
Experience with electronic charting
Experience with arranging community resources
Background in managing populations with complex medical or behavioral needs
