Job Description
The Field Care Coordinator at UnitedHealthcare will manage care for members with low-to-medium complexity needs, focusing on improving their medical, behavioral, and socioeconomic outcomes. The role requires significant fieldwork and collaboration with healthcare teams to create effective care plans, making a meaningful impact on community health.
Key Responsibilities
Engage members face-to-face and telephonically to complete comprehensive needs assessments
Develop and implement person-centered care plans
Collaborate with internal care teams, providers, and community resources
Provide education and coaching for member self-management
Support proactive discharge planning and manage care transitions
Advocate for members and families
Required Qualifications
2+ years of case management experience serving members with Serious Mental Illness (SMI)
1+ years of case management experience serving elderly and/or persons with physical or developmental disabilities
1+ years of experience with MS Office (Word, Excel, Outlook)
Driver’s License and access to reliable transportation
Preferred Qualifications
Bachelor’s degree in Psychology, Special Education, or Counseling
CCM certification
Experience working in team-based care
Experience in Managed Care
Social work experience
Bilingual
