Job Description
Molina Healthcare is seeking a Field Family Nurse Practitioner to deliver preventive primary care services in comfortable settings like homes and clinics. The role involves comprehensive patient assessments, medical care coordination, and community engagement to improve health outcomes.
Key Responsibilities
Provide general medical care and care coordination to various patient levels
Perform comprehensive evaluations and address chronic and acute complaints
Establish and document medical diagnoses and create treatment plans
Schedule patient appointments and coordinate post-discharge care
Perform in-person visits in various settings, including homes and nursing facilities
Participate in community-based Pop Up Clinics
Drive to various locations, maintaining cross-state licensing as needed
Collaborate with fellow nurse practitioners on best practices
Prescribe medications and perform necessary procedures
Document medical records in an electronic system
Required Qualifications
Master's degree in family health from accredited nursing program
Active and unrestricted national certification from recognized organizations
Current state-issued license to practice as a Family Nurse Practitioner
Current Basic Life Support for Healthcare Professional certification
Current unrestricted driver’s license
Advanced computer skills, proficient with Word, Excel, and Electronic Medical Record
Preferred Qualifications
3-5 years experience as a Registered Nurse and/or Nurse Practitioner in home health or community health
Experience managing chronic conditions in home health
Experience with underserved populations facing socioeconomic barriers
