Key Responsibilities
Identify ongoing needs and services of residents
Ensure proper follow through and documentation for residents
Conduct resident evaluations and gather information at specified times
Conduct monthly wellness visits for all residents
Monitor and follow up on residents who experience incidents
Maintain communication with resident's family and responsible party
Ensure weights and vital signs are obtained monthly
Maintain wellness files according to policies and regulations
Provide clinical support to community team members
Monitor residents' medication and treatment profiles
Participate in quality assurance meetings and clinical meetings
Required Qualifications
Graduate of approved college/school of nursing
Current state license as a professional LPN/LVN
Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment
Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care
Good assessment skills
Ability to handle multiple priorities
Effective written and verbal communication skills
Organizational and time management skills
Good judgment, problem-solving, and decision-making skills
Proficiency in computer skills, Microsoft Office, and Sunrise applications
Ability to work flexible hours
