Job Description

Key Responsibilities
Participate in Resident Care conferences and maintain Care Plans
Identify and respond to signs and symptoms of residents
Communicate resident status to nursing staff, physicians, and families
Maintain resident privacy and confidentiality
Document electronically
Prepare, administer, and document medications
Report resident concerns and incidents
Coordinate services during shifts
Complete required onboarding trainings
Perform additional assigned duties
Required Qualifications
Colorado State Licensed Practical Nurse license in good standing
Ability to work well with different teams
1-year experience in a long-term care facility
Must have computer word processing and database experience
Must be able to read, write and speak the English language
Must possess excellent customer service skills
Preferred Qualifications
1-year experience coordinating care in a clinical environment
Experience in caring for residents with dementia
IV Certification
1-year experience using electronic medical record platforms
