Job Description
Join The Jewish Board as a Case Manager to support adults with serious mental illness in achieving independent living. You'll work closely with residents, advocate for their needs, and help them develop essential life skills in a compassionate and culturally competent environment.
Key Responsibilities
Carry a caseload and practice person-centered and collaborative service planning
Evaluate and assess independent living skills
Advocate on behalf of residents with other service providers
Complete regular and timely documentation of all resident interactions
Participate in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
