Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Collaborate with interdisciplinary teams to assess care levels and develop care plans
Utilize therapeutic methods in accordance with individualized care plans
Monitor intervention progress and revise plans as needed
Provide education to patients and families on health maintenance and treatment adherence
Deliver direct patient care including case management and crisis intervention
Maintain timely and patient-centered documentation
Participate in administrative and development activities
Required Qualifications
Master's Degree in Social Work, Counseling, Marriage and Family Therapy, or Related Human Service Field
Previous experience in a healthcare setting
LCSW, LPC, LMFT, LADC
Valid Driver's License
Computer proficient
Ability to assess problems, determine solutions, and make decisions in a timely manner
Utilize supervision and team approaches in patient assessment and clinical interventions
Excellent communication skills
Preferred Qualifications
Knowledge of health-related social needs and cultural competence
Experience providing clinical social work interventions to all patient populations
