Job Description
Join The Jewish Board as a Case Manager, where you'll empower adults with serious mental illness to achieve independence and stability. You'll lead complex cases, provide personalized support, and advocate for residents while working collaboratively within a dedicated team.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Supports residents in medication management independence
Advocates on behalf of residents with service providers
Completes documentation of all resident interactions
Participates in planning and execution of events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
