Join ECHN as a Population Health Nurse, where you'll play a critical role in enhancing patient care for vulnerable populations through effective clinical workflow management and collaboration with healthcare teams.
Key Responsibilities
Collaborate with physicians and practice staff to identify vulnerable patients
Support ECMP physician practices without LPN staff
Develop individualized treatment care plans
Review and implement PCMH required processes
Participate in departmental and organizational committees
Required Qualifications
Graduate of an accredited school of Nursing
Current license as a Licensed Practical Nurse in Connecticut
Minimum of five years of nursing experience in a community practice health setting
Preferred Qualifications
Bachelor's degree in an applicable field
Strong analytical, organizational, and interpersonal skills
Working knowledge of Microsoft Office products
