Job Description
Join Sunshine State Health Plan as a Case Manager, where you'll play a crucial role in transforming healthcare for 28 million members. You'll develop personalized care plans, coordinate services, and provide essential support to improve health outcomes.
Key Responsibilities
Develop and facilitate care management activities for members
Evaluate member needs and recommend care plans
Coordinate care between members, families, and providers
Monitor member progress and update care plans as needed
Provide education to members and their families on healthcare options
Document care management activities for compliance
Facilitate access to community resources
Perform outreach to assess member needs
Required Qualifications
Degree from an Accredited School of Nursing
Bachelor's degree in Nursing
2 – 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State Licensure
Preferred Qualifications
Minimum of two years of pediatric experience (for Florida's Children's Medical Services)
