Job Description

Key Responsibilities
Assess, plan, and implement individualized care strategies
Identify and initiate referrals for social service programs
Manage care plans and communicate with stakeholders
Advocate for patients and families
Make outbound calls and receive inbound calls to assess health status
Required Qualifications
RN in the state of Texas or equivalent license (LPC, LBSW, LMSW, LSCW)
2+ years of experience with Intellectual and Developmental Disability (IDD)
2+ years of experience in community health or healthcare role
Intermediate proficiency in MS Word, Excel, and Outlook
Access to a designated quiet workspace with secure PHI handling
Ability to travel up to 75% in assigned region
Reliable transportation and valid US driver's license
Preferred Qualifications
1+ years of experience with long-term care services and support, Medicaid, or Medicare
Ability to create, edit, save, and send documents, spreadsheets, and emails
Knowledge of federal and state requirements for home and community-based settings
Reside within commutable distance of assigned duties
