Job Description
The Case Manager at The Jewish Board's Adult Residential Division supports adults with serious mental illness, guiding them towards independence and recovery through personalized service plans and advocacy. This role involves working closely with residents, managing complex cases, and ensuring their safety and well-being in a supportive environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates independent living skills and supports residents in meeting individual goals
Advocates on behalf of residents with other service providers
Completes documentation of resident interactions and ensures progress notes meet billing requirements
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
