Position Overview

We are seeking an experienced Registered Nurse Case Manager for a 13-week travel assignment. The Case Manager will be responsible for care coordination, discharge planning, and utilization review in a fast-paced acute hospital setting. This role requires the ability to work independently while collaborating with interdisciplinary teams to ensure safe, effective, and timely patient care transitions.

Key Responsibilities

• Perform care coordination and discharge planning for acute hospital patients (ratio 1:20).
• Utilize Epic EMR for documentation and case management tracking.
• Conduct patient needs assessments and arrange DME, SNF placement, and ambulance transportation.
• Collaborate in family meetings and treatment planning sessions.
• Ensure compliance with CMS, HIPAA, DRG, and Joint Commission standards .
• Work with HMOs, IPAs, and insurance providers to manage patient benefits and approvals.
• Participate in interdisciplinary team meetings and maintain effective communication with providers, social workers, and therapy services.

Requirements

• Active RN License (CA) – Required.
• Minimum 2 years acute hospital Case Manager experience (care coordination & discharge planning in a fast-paced environment).
• Travel nursing experience – Required.
• BLS Certification – Required.
• Epic EMR proficiency – Required.
• Supervisory references (2) from within the past 3 years in a comparable setting.
