Job Description
Join Sunshine State Health Plan as a Licensed Practical Nurse, where you will lead the charge in transforming the health of communities by developing personalized care plans for members with mental and behavioral health needs. Key Responsibilities
Develop and assess care management activities for mental and behavioral health needs
Evaluate member needs and recommend care plans
Perform outreach to assess member needs
Develop ongoing care plans for high acuity members
Coordinate services between members, families, and care providers
Monitor care plans and make recommendations as needed
Educate members and families on healthcare options and benefits
Provide feedback to leadership on care improvement opportunities
Required Qualifications
Master's degree in Behavioral Health or Social Work
2 – 4 years of related experience
Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN
Preferred Qualifications
Experience in pediatric care
Ability to travel locally up to 80%
