Job Description
As a Case Associate at The Jewish Board, you'll play a vital role in client care by developing action plans and utilizing community resources. This position involves conducting home visits, providing counseling, and ensuring clients receive the necessary support to thrive.
Key Responsibilities
Develop action plans for client care
Conduct routine community contacts and home visits
Locate and utilize community resources for clients
Provide structure and guidance in independent living skills
Evaluate clients for individualized treatment programs
Perform individual/group counseling
Manage case documentation and notes
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
