Join Optum as an RN Healthcare Manager, where you'll support patients with high-risk health conditions and collaborate with multidisciplinary teams to enhance patient care and quality of life.
Key Responsibilities
Identify patients at high risk
Support longitudinal care of patients with chronic conditions
Perform assessments and medication reconciliations
Conduct Motivational Interviewing and Self-Management Goal setting
Provide patient education and create referrals
Support transitions from inpatient to outpatient settings
Coordinate with providers to establish or update plans of care
Document accurately in electronic medical records
Participate in daily huddles and Patient Care Coordination meetings
Prepare reports for weekly meetings
Maintain continued competence in nursing practice
Perform clinical tasks within their scope of practice
Perform other related duties as assigned
Required Qualifications
Bachelor's degree in Nursing or Associate's degree in Nursing with four or more years of experience
Registered Nurse with an active and unrestricted license to practice in the state of employment
Current BLS certification
2+ years of experience in a physician's office, clinical or hospital setting
Proven knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF and IHD
Proven excellent verbal and written skills
Proven solid interpersonal skills
Ability to interact productively with individuals and with multidisciplinary teams
Proficient computer skills to work efficiently with electronic medical records
Proven excellent organizational and prioritization skills
Driver's License and access to reliable transportation
Tuberculosis screening and proof of immunity to certain diseases
Preferred Qualifications
Experience related to patient education and/or motivational interviewing skills and self-management goal setting
Fluent written and verbal skills in English and Spanish
