Job Description
Join The Jewish Board as a Senior Case Manager, where you'll empower adults with serious mental illness to achieve independence in a supportive community. You'll lead complex case management efforts, advocate for residents, and facilitate their recovery journeys while working within a compassionate team.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Advocates for residents with other service providers
Completes regular documentation of resident interactions
Facilitates resident support groups and meetings
Participates in planning and execution of events
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
