Job Description

Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate with stakeholders to ensure quality care
  • Advocate for patients and families
  • Assess, plan, and implement individualized care strategies
  • Identify barriers to care and provide management interventions
  • Initiate referrals for social service programs
  • Provide resource support for local services
  • Manage person-centered service/support plan
  • Conduct home and in-person visits
  • Document member information and care management activities

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or Master's degree with licensure as a Social Worker (LMSW, LCSW, LLMSW) or Bachelor's degree with licensure (LLBSW, LBSW)
  • 2+ years of experience in community health setting
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience with long-term care needs
  • 1+ years of experience with electronic documentation systems and MS Office
  • Access to reliable transportation for member visits (up to 75% travel)
  • Access to a designated quiet workspace at home
  • Ability to travel for quarterly meetings
  • Must reside within Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience with Motivational Interviewing and Trauma-Informed Care
  • Experience serving individuals with co-occurring disorders
  • Experience with MI Health Link (MMP)
  • Experience in Managed Care
  • Knowledge of NCQA documentation standards
