Job Description

Key Responsibilities
Develop and implement care plan interventions
Communicate with stakeholders to ensure quality care
Advocate for patients and families
Assess, plan, and implement individualized care strategies
Identify barriers to care and provide management interventions
Initiate referrals for social service programs
Provide resource support for local services
Manage person-centered service/support plan
Conduct home and in-person visits
Document member information and care management activities
Required Qualifications
Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan or Master's degree with licensure as a Social Worker (LMSW, LCSW, LLMSW) or Bachelor's degree with licensure (LLBSW, LBSW)
2+ years of experience in community health setting
1+ years of experience with local behavioral health providers
1+ years of experience with long-term care needs
1+ years of experience with electronic documentation systems and MS Office
Access to reliable transportation for member visits (up to 75% travel)
Access to a designated quiet workspace at home
Ability to travel for quarterly meetings
Must reside within Michigan
Preferred Qualifications
RN or LMSW; LCSW, LLMSW
1+ years of medical case management experience
Experience with Motivational Interviewing and Trauma-Informed Care
Experience serving individuals with co-occurring disorders
Experience with MI Health Link (MMP)
Experience in Managed Care
Knowledge of NCQA documentation standards
