Key Responsibilities
Conduct comprehensive bio-psychosocial assessments and establish care plans
Provide direct patient care including case management and emotional support
Refer patients to appropriate levels of care and resources
Maintain timely patient-centered documentation
Contribute to improved patient experience as a team member
Provide social work field supervision to second year social work students
Required Qualifications
Master’s degree in social work, Counseling, Marriage and Family Therapy or related Human Service field
Current Connecticut LCSW, LPC, LMFT, or LADC
Minimum of two years’ experience in a psychiatric setting
Highly effective verbal and written communication skills
Valid Driver's License
Preferred Qualifications
Previous experience in a healthcare setting
