Job Description
Join The Jewish Board as a Case Manager, where you will support adults with mental illness in achieving their recovery goals and enhancing their independence. Utilize your skills to advocate for residents, manage complex cases, and contribute to a culturally competent and supportive environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Ensures service plans are completed according to guidelines
Advocates for residents with service providers
Completes documentation of resident interactions
Participates in intake, assessments, and follow-ups
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
