• Job Description: -
   RN who will be responsible for the care of patients in their home.
• The candidate will have 2 years of experience performing OASIS assessments and care planning for in-home patients.
• Previous experience with our EMR, Meditech, is a plus.
• person will also be in the call rotation for the department, one in four.
• The scheduled shifts are Monday through Friday. We are open 32-40 hours per week.
