Job Description
Join The Jewish Board as a Case Manager and support adults with serious mental illness in achieving independence. You will lead complex cases, advocate for residents, and facilitate essential living skills, all while fostering a compassionate and inclusive environment.
Key Responsibilities
Carries a caseload and practices person-centered service planning
Evaluates and assesses independent living skills
Advocates for residents with service providers
Completes documentation of resident interactions
Participates in planning special and holiday events
Facilitates support groups and meetings
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
