Join Molina Healthcare as an IRIS Consultant, a remote role where you will make a meaningful impact on the lives of those with long-term care needs. You'll partner with individuals enrolled in the Wisconsin IRIS program, helping them navigate resources and achieve their personal goals with compassion and creativity.
Key Responsibilities
Meet in person with IRIS participants a minimum of four times per year
Provide program orientation to new participants
Explore the participant's life goals and connections
Assist in identifying personal outcomes and ensure they are met
Document all orientation and planning activities
Research community resources for participants
Educate participants on budget reports and financial management
Develop trusting relationships and communicate effectively
Maintain confidentiality and HIPAA compliance
Collaborate with other staff for successful implementation of plans
Attend monthly team meetings and weekly check-ins
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or closely related field
1+ year of direct experience delivering social services to target groups
Ability to work independently with minimal supervision
Knowledge of Long-Term Care programs
Excellent problem-solving and critical thinking skills
Strong time management and prioritization skills
Ability to develop and maintain professional relationships
Resourceful with knowledge of community resources
Proficient in various technologies including Outlook, Skype, Teams, PowerPoint, Excel, Word
Preferred Qualifications
Experience working with individuals with intellectual or physical disabilities and older adults
Familiarity with principles of self-determination
