Key Responsibilities
Function as a case planner to supply appropriate interventions for client care
Develop action plans and utilize existing resources for client care
Conduct routine community contacts and evaluate clients' living conditions
Provide case management and individual/group counseling
Perform community outreach and establish linkages with supportive groups or organizations
Document necessary notes and assessments
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
