Job Description
Join BoldAge PACE as an RN Care Manager and enhance the quality of life for seniors through personalized care. Key Responsibilities
Assess care needs of participants and provide nursing and healthcare interventions
Collaborate with interdisciplinary team to develop care plans
Provide nursing care in various settings including participants’ homes
Participate in 24/7 on-call process for participant triage
Monitor participants' acute and chronic care needs
Teach participants and caregivers about self-care and healthy lifestyles
Participate in interdisciplinary team meetings and quality improvement projects
Required Qualifications
State RN License required
1 year experience providing care as an RN required
Preferred Qualifications
Bachelor of Science in Nursing Degree preferred
1 year of experience working with a frail or elderly population preferred
Experience in home care, long-term care and/or managed care preferred
